Sample Online Application
This sample is meant only as an example of the fields you will encounter in the CCF online application. Application packages must be completed and submitted online by April 14, 2021 at noon. ![]()

Sample SME Evaluation Form
Subject matter experts, or SMEs, conduct the second stage of the application review process, following CIT's administrative compliance assessment. SMEs will evaluate each application using this form, which ties to the evaluation criteria outlined on page 7 of the guidelines. |
